Carbonic anhydrase inhibitor
Each tablet contains
Acetazolamide 250 mg
Cidamex contains Acetazolamide which is a potent specific inhibitor of carbonic anhydrase ; the enzyme that catalyzes the reversible action involving hydration of carbon dioxide and dehydration of carbonic acid , this inhibitory action leads to :
In eye : Decreases the secretion of aqueous humor and thus results in a drop
in intra-ocular pressure .
In CNS : Retardation of abnormal , paroxysmal , excessive discharge from CNS neurons .
In Kidney : Renal loss of HCO3 ion , which carries out sodium , potassium and water leading to promotion of alkaline diuresis in instances of abnormal fluid retention .
Indications & usage :
Open angle glaucoma , secondary glaucoma and preoperatively in angle closure glaucoma .
A second line drug for both tonic –clonic and partial seizures and occasionally
helpful in atypical absence , atonic and tonic seizures and center encephalic
As adjunctive treatment of edema due to congestive heart failure and drug
induced edema .
As prophylaxis therapy against mountain sickness .
Hypokalemia , hyponatremia , hyperchloraemic acidosis .
Renal and severe hepatic impairment , suprarenal gland failure .
Hypersensitivity to sulfonamides .
Long term treatment in patients with chronic non-congestive angle closure glaucoma .
Side effects :
GIT disturbances ( e.g. nausea , vomiting , diarrhea , loss of appetite …..) .
On prolonged administration : headache , fatigue ,irritability , flushing , dizziness , polyuria , reduced libido or depression may occur .
Also on long term therapy : metabolic acidosis and electrolyte disturbance may occur which can be corrected by administering potassium bicarbonate .
Being a sulfonamide derivative , blood disorders including agranulocytosis , thrombocytopenia , urticaria and photosensitivity may rarely occur .
Drug interactions :
- Acetazolamide may enhances the effect of other drugs such as amphetamines
,ephedrine , and quinidines .
- It also reduces the effect of methenamine ,and its compounds .
- Acetazolamide may enhances anti-epileptic induced osteomalacia
- Used of Acetazolamide with aspirin may results in severe acidosis , and increases CNS toxicity .
Warning & precautions :
If used for prolonged period , monitor blood count and plasma electrolyte concentrations .
Used cautiously in pulmonary obstruction ( risk of acidosis ) , elderly , pregnancy and breast – feeding .
Used cautiously in patients receiving concomitant high doses of aspirin .
Dosage & administration :
Usual dose : 1-4 tablets daily in divided doses according to the case or as prescribed by the physician .
For epilepsy in children : ( 8-30 ) mg/ kg daily , maximum 750 mg daily .
